Their dedicated team of Healthcare professionals work collaboratively to offer personalized care tailored to each individual’s needs.
They are in search of Well-Visit Nurse Practitioners in both Maplewood and Cherry Hill, New Jersey. The ideal candidate will have a strong background in geriatrics or home care, bringing expertise and empathy to the role. You will be responsible for conducting well-visits, managing chronic conditions, and collaborating with our multidisciplinary team to enhance the health and well-being of our homebound patients.
The annual base salary range is $120,000 to $155,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Responsibilities:
- Conduct Well-Visits: Perform thorough physical examinations, health assessments, and routine screenings for elderly patients during home visits
- Develop, implement, and evaluate individualized care plans. Monitor chronic conditions, medication regimens, and provide education on disease management
- Work closely with physicians, nurses, social workers, and other healthcare professionals to ensure coordinated and comprehensive care.
- Provide education to patients and their families on health maintenance, disease prevention, and managing chronic conditions
- Coordinate and manage follow-up visits, laboratory tests, and referrals to specialists as needed
- Identify and respond to medical emergencies during home visits, providing initial care and coordinating further treatment as necessary
Requirements:
- Master’s degree in Nursing from an accredited Nurse Practitioner program
- Current and valid Nurse Practitioner license in NJ
- Certification in Adult-Gerontology Primary Care or Family Nurse Practitioner preferred
- Minimum of 1 year of experience in geriatrics or home care settings
- Compassionate approach with a focus on patient-centered care
