About the Opportunity
Our client is seeking a compassionate and experienced Licensed Clinical Social Worker (LCSW) to join their managed care team. This hybrid role involves conducting case management primarily through telehealth and office-based activities, with field visits 2-3 times per month to support the geriatric population. The LCSW will provide comprehensive case management services, including assessments, care coordination, and advocacy, to improve the quality of life and health outcomes for elderly clients.
The annual base salary starts at $65,000 – $90,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Responsibilities
- Conduct home visits or field assessments 2-3 times per month to assess living conditions and ensure clients’ needs are being met
- Utilize telehealth platforms for regular client check-ins, consultations, and follow-ups
- Connect clients with community resources, support services, and benefits, including housing, transportation, nutrition, and financial assistance
- Maintain accurate and timely documentation in the company’s case management system
- Ensure compliance with all relevant regulations, company policies, and best practices in geriatric care and case management
- Provide ongoing case management services, including monitoring client progress, adjusting care plans, and coordinating services such as medical care, mental health services, and social supports.
- Develop and implement individualized care plans in collaboration with clients, their families, and healthcare providers
- Conduct comprehensive biopsychosocial assessments of geriatric clients
Requirements
- Master’s degree in Social Work from an accredited institution
- Licensed Clinical Social Worker (LCSW) in NJ.
- Minimum of 1 year of experience in case management or clinical social work, preferably with the geriatric population
- Proficient in using telehealth platforms, case management software, and electronic health records
