Pediatric Nurse Case Manager

About the Opportunity

Our client, a nonprofit organization in New York City, is seeking a compassionate and skilled Pediatric Nurse Case Manager to coordinate and manage the healthcare needs of pediatric patients in the Bronx. The ideal candidate will work closely with families, medical providers, and multidisciplinary teams to ensure children receive comprehensive, high-quality care. This role involves care planning, patient advocacy, and ensuring continuity of care for children with acute or chronic conditions.

The annual base salary range is $85,000 to $105,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Develop and implement individualized care plans for pediatric patients based on medical, social, and developmental needs
  • Collaborate with physicians, therapists, social workers, and community resources to ensure seamless care
  • Monitor and adjust care plans as needed to promote optimal health outcomes
  • Provide education and support to families regarding treatment options, medications, and home care
  • Serve as a liaison between families, healthcare providers, and insurance companies to ensure access to care
  • Assist families in navigating insurance authorizations, Medicaid/Medicare services, and community resources
  • Advocate for early intervention services, special education needs, and therapy referrals as necessary
  • Provide crisis intervention and emotional support for families dealing with complex medical conditions
  • Maintain accurate and up-to-date medical records, care plans, and progress notes
  • Ensure compliance with HIPAA, Medicaid, and pediatric healthcare regulations
  • Participate in quality improvement initiatives to enhance patient outcomes and care delivery

 

Job Requirements

  • Active RN license in NY
  • Case management certification (CCM or CPN) is a plus
  • Minimum 1 year of pediatric nursing experience (hospital, home health, or clinic setting preferred)
  • Fluency in Spanish is required
  • Strong knowledge of pediatric conditions, developmental milestones, and family-centered care
  • Experience working with children with special needs, chronic illnesses, or medically complex cases
  • Excellent communication, problem-solving, and advocacy skills
