Primary Care Family Nurse Practitioner (Spanish-Speaking)

About the Opportunity

We are seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our client’s team in New York City and provide primary care services to union workers and their dependents. The FNP will play a crucial role in delivering preventive care, managing chronic conditions, and promoting overall health and wellness. The annual base salary range is $140,000 to $150,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Conduct comprehensive health assessments, including medical history, physical exams, and screenings, to identify health needs and risks
  • Provide evidence-based primary care services to patients of all ages, including diagnosis, treatment, and management of acute and chronic conditions
  • Develop and implement individualized care plans in collaboration with patients, incorporating their preferences, values, and cultural beliefs
  • Educate patients and their families about preventive health measures, lifestyle modifications, and self-management strategies to optimize health outcomes
  • Order and interpret diagnostic tests and procedures as needed, and make appropriate referrals to specialists for further evaluation and treatment
  • Document patient encounters accurately and timely in the electronic health record (EHR) system, ensuring compliance with regulatory standards and organizational policies
  • Collaborate with interdisciplinary team members, including physicians, nurses, and ancillary staff, to coordinate care and ensure continuity of services

 

Job Requirements

  • Master’s or Doctoral degree in Nursing with specialization as a Family Nurse Practitioner (FNP)
  • Current certification as a Family Nurse Practitioner (FNP) and unrestricted license to practice in New York
  • At least 1 year of experience providing primary care services to patients of all ages, preferably in a community health or ambulatory care setting
  • Spanish fluency required
  • Strong clinical assessment and diagnostic skills, with proficiency in evidence-based practice guidelines
  • Excellent communication and interpersonal skills, with a patient-centered approach to care
  • Ability to work independently as well as collaboratively within a multidisciplinary team
