About the Opportunity
We are seeking a compassionate and experienced Registered Nurse (RN) to join our client’s team in the role of Safe Options Support RN. This position will play a crucial role in providing healthcare support and outreach services to individuals experiencing homelessness. The ideal candidate will have a strong background in community health, a deep understanding of the unique healthcare needs of the homeless population, and a commitment to improving the health and well-being of vulnerable individuals.
The annual base salary range is $90,000 to $104,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Conduct outreach to homeless individuals in various settings, including shelters, streets, and community centers
- Build trusting relationships with clients and provide support to encourage engagement with healthcare services
- Perform comprehensive health assessments for individuals experiencing homelessness, addressing immediate healthcare needs and identifying underlying health concerns
- Collaborate with healthcare providers and social service agencies to coordinate ongoing care
- Deliver health education and preventive care information to clients, addressing topics such as hygiene, nutrition, and the management of chronic conditions
- Advocate for and facilitate access to relevant health resources and services
- Implement harm reduction strategies and provide education on safe practices related to substance use and other health risk behaviors
- Distribute and educate clients on harm reduction supplies as needed
- Work collaboratively with outreach teams, social workers, and other community partners to ensure a coordinated and holistic approach to supporting homeless individuals
- Participate in case conferences and multidisciplinary meetings to discuss client care plans
- Maintain accurate and up-to-date electronic health records for each client encounter
- Document outreach activities, health assessments, and follow-up care plans
Job Requirements
- Current and valid Registered Nurse (RN) license
- Experience in community health, outreach, or public health nursing
- Knowledge of and sensitivity to the unique healthcare needs and challenges faced by individuals experiencing homelessness
- Strong interpersonal and communication skills
- Ability to work independently and collaboratively in a dynamic and diverse community setting
