Director of Case Management (RN)

About the Opportunity

Our client in the Bronx, NY is in search of an RN Director of Case Management to oversee and guide their case management team, ensuring the delivery of high-quality, patient-centered care while optimizing resource utilization and improving health outcomes.This hybrid position requires a seasoned professional with a strong background in managed care or case management.

The annual base salary range is $120,000 to $150,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.

 

Job Responsibilities

  • Lead and manage the case management department, including hiring, training, supervising, and evaluating staff
  • Develop and implement case management strategies, policies, and procedures to ensure effective patient care coordination and resource management
  • Oversee the assessment, planning, implementation, and evaluation of individualized care plans for patients, ensuring alignment with organizational goals and regulatory requirements
  • Collaborate with interdisciplinary teams, including physicians, nurses, social workers, and other healthcare professionals, to facilitate integrated and coordinated care
  • Monitor and analyze key performance indicators (KPIs) and metrics to identify trends, opportunities for improvement, and areas requiring corrective action
  • Ensure compliance with all federal, state, and local regulations, as well as accreditation standards related to case management
  • Manage and optimize relationships with managed care organizations, insurance providers, and other external stakeholders
  • Provide education, training, and support to case management staff to enhance their professional development and performance
  • Utilize data and analytics to drive decision-making and improve case management processes and outcomes
  • Participate in quality improvement initiatives, committees, and projects to advance the effectiveness and efficiency of case management services

 

Job Requirements

  • Registered Nurse (RN) with a current and unrestricted license in New York.
  • Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing, Healthcare Administration, or related field preferred.
  • Minimum of 1 year of experience in case management, managed care, or a related field, with at least 2 years in a leadership or supervisory role.
  • MDS Experience preferred.
  • Strong knowledge of case management principles, managed care practices, and healthcare regulations.
  • Excellent leadership, communication, and interpersonal skills.
  • Ability to analyze complex data, develop actionable insights, and drive process improvements.
  • Ability to work independently and as part of a team in a hybrid work environment.
