About the Opportunity
Our client is social services organization with over 50 years of experience serving the populations throughout New York City. They are in search of a Family Team Conference Coordinator/Facilitator to be responsible for coordinating and facilitating Family Team Conferences. This includes Service Termination Conferences at 3 different intervals per year on a caseload of 96 families.
The annual base salary range is $60,000 to $65,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Track and Schedule all Family Team Conferences (FTC’s)
- Document FTC’s in connections
- Facilitate Case Planning FTC’s
- Facilitate 45-day FTC’s at the direction of the Program Director
- Facilitate Service Termination Conferences at the direction of the Program Director
- Attend and actively participate in Mobility Mentoring Training, Apricot Data Base training and ACS training as required
- Responsible for Data Entry regarding Smart Goals Achieved
- Monitor and Assess FTC Timelines
- Review cases prior to facilitating the FTC
- Review Mobility Mentoring Bridge and Smart Goals prior to facilitating the FTC
- Review FASP outcomes in preparation for an FTC
- Participate in outcome data collection as directed by Program Director
- Lead Mobility Mentoring Data Collection
- Monitor Programmatic Outcomes
- Monitor Fidelity to Program Model with the management team as directed by Program Director.
- DATA Entry as assigned
- Other duties may be assigned as needed or as program evolves as directed by Program Director
Job Requirements
- MSW or MA in a related field
- Knowledge of Child Welfare System preferred
- Excellent Group Work and/or Family Counseling skills
- Excellent computer skills (including Microsoft Suite)
- Excellent oral and written communication skills
- Ability to interact with a multicultural team
- Strong ability to conduct presentations to staff and families
