About the Opportunity:
The RN, Patient Care Coordinator position provides coordination of care for patients who have heart transplants in a part-time capacity. Key responsibilities include immunosuppression monitoring, prophylactic antibiotic management, and monitoring for organ rejection and infection. This role ensures all post-heart transplant patient care is coordinated across the continuum and involves frequent communication with multidisciplinary team members, patients, and families.
The estimated base pay for these positions are $60.50 to $71.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conduct specific assessments and create care plans for heart transplant recipients.
- Provide education on immunosuppressive therapies and prophylactic antibiotic management.
- Collaborate with attending MD, APP, Pharm D, and other healthcare professionals to coordinate care.
- Monitor and document patient progress, including graft function and general health status.
- Communicate changes in patient conditions that may require immediate action.
Job Requirements:
- Valid CA RN License.
- AHA BLS certification.
- Minimum 1 year of experience in ambulatory care.
- Experience in cardiac nursing, specifically with heart failure and/or heart transplant recipients.
- Preferred experience in the management of other organ transplant recipients.
- Epic charting system experience.
