About the Opportunity:
This is a full-time contract position as an RN Patient Care Coordinator in San Francisco, CA. The role involves coordination of care for patients who have heart transplants, including immunosuppression monitoring, prophylactic antibiotic management, monitoring for organ rejection, and infection monitoring. The position ensures all routine and typical patient monitoring takes place efficiently and thoroughly.
The estimated base pay for these positions are $60.50 to $71.50 per hour. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities:
- Conducting immunosuppression monitoring and prophylactic antibiotic management.
- Monitoring for organ rejection and infection.
- Collaborating with multidisciplinary team members regarding patient care issues.
- Facilitating patient activities with consulting departments and physicians.
- Assisting other nurses and staff in planning, implementing, and evaluating the care of transplant patients.
- Assessing patient and family educational and other needs, coordinating with relevant departments like financial counseling, social work, and nutrition.
Job Requirements:
- Current CA RN License and AHA BLS certification.
- Minimum of 1 year of experience in ambulatory care.
- Cardiac nursing experience with heart failure or heart transplant management.
- Ability to float to other facilities within the network.
- Preferred experience in managing other organ transplant recipients and working knowledge of immunosuppressive therapies.
- Epic charting system experience required.
