About the Opportunity
Our client, a mission-driven healthcare and social services organization, is seeking an Executive Director of Clinical Programs to provide clinical leadership to their team in Suffolk County, NY. This executive-level role is responsible for shaping clinical strategy, strengthening partnerships across healthcare and social service systems, and advancing integrated care models that address medical needs alongside social determinants of health.
Please note: the individual in this role must have an active Nurse Practitioner license in New York.
The annual base salary range is $135,000 to $160,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Serve as the clinical leader for a multi-partner care network, driving integration between medical providers, health plans, and community-based organizations
- Design and advance clinical frameworks, policies, and operational standards that support effective collaboration with healthcare and payer partners
- Identify and develop new programmatic opportunities focused on prevention, chronic disease management, wellness, and evidence-based interventions
- Build and maintain strong relationships with healthcare stakeholders, including primary care practices, FQHCs, specialty groups, and managed care organizations
- Guide innovation at the intersection of primary care and social services, with a focus on improving outcomes and reducing disparities
- Support data-driven validation of services delivered, including monitoring progress, identifying changes in participant status, and guiding timely interventions
- Monitor healthcare policy, payment reform, and market trends at the local, state, and national levels to inform organizational strategy
Job Requirements
- Must be a Nurse Practitioner with an active New York State NP license
- Clinical background in primary care strongly preferred
- Demonstrated understanding of value-based payment models and healthcare financing
- Experience evaluating program outcomes, quality metrics, and return on investment
- Knowledge of regional demographics, community health needs, and health disparities
- Experience supporting advocacy, community engagement, or public-facing initiatives
- Ability to collaborate effectively with executive leadership and cross-functional teams
