About the Opportunity
Our client is seeking a dedicated Social Worker, MSW to join their Supportive Housing Program located in Brooklyn, New York. The Social Worker will provide comprehensive case management and support services to residents in their supportive housing program, helping them achieve housing stability, enhance their quality of life, and access community resources. This role involves conducting assessments, developing care plans, and collaborating with a multidisciplinary team to address the unique needs of each resident.
The annual base salary starts at $50,000 – $60,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Responsibilities
- Conduct detailed biopsychosocial assessments
- Develop and implement individualized care plans in collaboration with residents
- Assist residents in accessing essential services such as healthcare, mental health support, substance abuse treatment, financial assistance, and employment resources
- Provide immediate support and intervention in crisis situations, ensuring residents have access to emergency services and stabilization resources
- Connect residents with community resources, including social services, legal aid, educational opportunities, and recreational activities
- Support residents in developing life skills such as budgeting, time management, and problem-solving
- Work closely with other program staff, housing providers, healthcare professionals, and community organizations to ensure a coordinated approach to resident care
- Build trusting relationships with residents through regular check-ins, support, and engagement activities to promote a positive and supportive living environment
- Participate in training, supervision, and team meetings
Requirements
- Master’s degree in Social Work from an accredited institution
- Licensed Master Social Worker or eligibility for licensure
- Minimum of 2 years of experience in social work, case management, or supportive housing services
- Understanding of supportive housing principles, resident support needs, and community resources
