About the Opportunity
We’re seeking a dedicated Clinical Coordinator to join a mobile, multidisciplinary Assertive Community Treatment (ACT) team serving adults across New York City. The role offers a meaningful opportunity to support individuals living with serious mental illness and co-occurring substance use disorders, helping them build stability, independence, and a stronger sense of well-being. This individual will be part of a compassionate, mission-driven team focused on person-centered, recovery-oriented care delivered directly in the community.
The annual base salary range is $65,000 to $80,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide hands-on care coordination and case management services in community-based settings
- Partner with clients to develop and maintain individualized care plans, adapting them as needs evolve
- Conduct ongoing clinical assessments and monitor progress toward treatment goals
- Deliver outreach and engagement services to support consistent participation in care
- Coordinate medical, behavioral health, housing, and social services to ensure holistic support
- Maintain accurate and timely documentation within electronic health records
- Participate in rotating after-hours on-call coverage
Leadership & Team Support Responsibilities

Job Requirements
- Master’s degree in a human services or behavioral health field
- Active or eligible New York State licensure (e.g., LMSW, LCSW, LMHC, LMFT, LCAT, or related)
- At least 1 year of experience in a community-based or mobile treatment program (ACT experience preferred)
- Strong understanding of serious mental illness (SMI) and substance use disorders (SUD)
- Excellent organizational, communication, and crisis management skills
- Comfort working in the field and engaging clients in diverse community settings
