About the Opportunity
We’re partnered with a respected, mission-driven community health organization seeking an experienced Family Nurse Practitioner (FNP) to join a collaborative primary care team serving patients in Brooklyn. This is an excellent opportunity for an NP who enjoys practicing comprehensive outpatient medicine while working alongside an experienced Medical Director and an interdisciplinary team dedicated to improving community health.
This full-time position is based at a partner clinical site near Greenpoint and offers the stability, benefits, and professional support of a well-established healthcare organization.
The annual base salary range is $135,000 to $155,000. Actual compensation offered to the successful candidate may vary from posted hiring range based upon geographic location, work experience, education, and/or skill level, among other things. Details about eligibility for bonus compensation (if applicable) will be finalized at the time of offer.
Job Responsibilities
- Provide comprehensive primary care services for patients across the lifespan in an outpatient setting
- Diagnose, treat, and manage acute illnesses, chronic conditions, and preventive healthcare needs
- Deliver inclusive, evidence-based care, including gender-affirming healthcare, HIV prevention and management, and routine wellness services
- Perform annual physicals, health screenings, and chronic disease management
- Order and interpret diagnostic testing and prescribe medications as appropriate
- Collaborate closely with physicians, nurses, behavioral health providers, nutritionists, social workers, and health educators to provide coordinated care
- Educate patients on disease prevention, wellness, and healthy lifestyle choices
- Maintain accurate and timely electronic medical records while adhering to quality and regulatory standards
- Build trusted relationships with patients while promoting equitable, culturally responsive healthcare
Job Requirements
- Active New York State Family Nurse Practitioner license
- Minimum of 2 years of post-graduate Family Nurse Practitioner experience in a Primary Care, Community Health, Family Medicine, or Ambulatory Care setting
- Bilingual Spanish language skills required
- Experience providing care to diverse patient populations is highly valued
- Strong clinical judgment, communication, and collaboration skills
Job Schedule
- Full-Time
- Monday through Friday
- Day Shift (9:00 AM – 5:00 PM)
- No routine evenings or weekends
